PCUMC Youth & Children Registration

For a more efficient start to our programs, we ask you to fill out this registration form for your children before their first attendance. This will help us better understand their needs and ensure a smooth and enjoyable experience for everyone.

This form must be filled out by a parent or guardian. By submitting this form, you confirm that the information on this form is accurate, and that you are a parent or guardian of the youth or child that this form is being submitted for.

Special note about Hot Chocolate with God & S'mores for kids:
Throughout our many years of this program, we have been blessed to see so many children and youth get a chance to build relationships with each other and God. While we never want to turn anyone away, we have space left for only a few grades. If you are interested in this program, please contact Adrienne Demmin for availability and registration before attending. Thank you.
